First Visit

Your initial appointment will consist of a consultation with Dr. Schwartz, explaining your diagnosis and treatment options. The doctor will discuss your medical history, go over any x-rays and treatment plans provided by your referring doctor and answer any questions. If x-rays or cone beam scans are needed, we can also take those at the initial appointment. Our staff will also provide an estimate at the consultation appointment for any treatment or surgical procedure that your referring doctor, yourself and the doctor determine that you require. In order for our staff to provide you with the most accurate information possible, please make sure that you bring all medical and dental insurance information to the initial consultation. Our staff can also provide you with information regarding Care Credit, if you require a payment alternative. 

Please keep in mind that oral surgery procedures performed by Dr. Schwartz require this initial consultation to discuss medical history, treatment and financial plans, as well as instructions for pre-operative care. After the initial consultation, our staff can assist you in scheduling your surgery appointment.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and any x-rays if applicable.
  • A list of medications you are presently taking. As well as all medical history information, such as previous surgeries, allergies and other medical information you feel is pertinent.
  • If you have medical or dental insurance, please make sure that you bring all insurance information with you to the initial consultation. Including insurance cards (if provided) and all Subscriber information, such as birth dates. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

Please make sure that you have all information at the initial consultation pertaining to any medical conditions that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.). It is most important that you also provide our office all of the names and correct dosage of any medications that you are currently taking (i.e., heart medications, aspirin, anticoagulant therapy, etc.) at the initial consultation appointment.

X-Rays

If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. Please keep in mind that Dr. Schwartz’s office is equipped with a state of the art Cone Beam 3D and 2D Panoramic Imaging System, as well as digital X-rays for any diagnostic images that you may require.
