For your convenience, we accept cash, personal checks, Visa, MasterCard, Discover, American Express and Care Credit. We deliver the finest care at the most reasonable cost to our patients, therefore the payment discussed at your initial consultation is due at the time service is rendered, unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at Daniel M. Schwartz D.M.D., MD & Michael S. Hauser D.M.D., M.D. Phone Number 216-464-1200. Many times, a simple telephone call can clear up any misunderstandings.
We will send you a statement after all charges have been submitted to your insurance company. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated. We can provide you with the information for Care Credit so you can apply for a monthly payment plan with them. Please keep in mind that any monthly payment plan that you require must be arranged between you and Care Credit prior to your scheduled surgical appointment.
Please understand that our office will submit all charges to your insurance as a courtesy to you. We will work with you and your insurance company to maximize your benefits, but we can only do so if we receive correct and complete insurance information from you. At your initial appointment, please make sure all insurance cards and information provided to our office is up-to-date, complete and correct.
Please remember that you are fully responsible for all fees not covered under your insurance.
